The WHF Forum on Multimorbidity and Integrated Care, convened in 2024, highlighted the limitations of disease-specific, siloed approaches to managing multiplelong-term conditions, and underscored the need for more coordinated,person-centred care systems.

Building on these discussions, the Roadmap was developed by the same expert group throughout 2025.

The successful adaptation and implementation of the Roadmap at country level will be helped by the continued engagement of this group.
